Handwritten applications will not be accepted by the Consulate. Please note that each page of your application MUST be printed SINGLE-SIDED only. Applications printed/signed double-sided will be rejected. B)Original, signed United States of America passport with at least 6 months of remaining validity and at least 2 consecutive blank visa pages. C)Passport-type photograph: 1. A copy of driver's license, state issued ID for the most recent month, showing the applicant's name and current address. E)Itinerary. • Application form. The application form must be signed by both parents.
• Birth certificate. A copy of the applicant's birth certificate.
• Parents' passport copies. A copy of the information pages of both parents' signed passports.
When applying for a child where one of the parents is a Chinese citizen, you must show evidence that the parent has a green card and has held Permanent Resident status since BEFORE the child was born.
If you cannot meet all the above requirements you will likely be required to appear in person at the Chinese Consulate to apply, with both parents and child present.
